YOUNG ADULTS IN TRANSITION
Staff provides support to young adults and their families...
- To facilitate in their transition from school to community life (including accommodation, work, leisure activities, and further education).
- To facilitate life planning and implementation of goals.
- To facilitate connections with the community
- To ensure appropriate supports and services.
- To provide education/skill training (individual and group) in:
- Pre-employment
- Social
- Daily living
- Literacy
Who is eligible?
- Young Adults, age 18-28 who have a developmental disability
- Must reside in Brant County and live at home
How do you access the transition program?
How will young adults benefit?
- Young adults with a developmental disability will be actively involved in their community following graduation from school.
- Young adults will make informed choices about their own life.
- Young adults will build a sense of competence in decision making, accessing resources in their community and self-advocacy.
