YOUNG ADULTS IN TRANSITION
 |
Staff provides support to young adults and their families...
- To facilitate connections to the community and building relationships.
- To facilitate life planning and implementation of goals.
- To facilitate in their transition from school to community life (including accommodation, work, leisure activities and further education).
- To coordinate appropriate supports and services.
- To provide education (individual and group) in:
- Pre-employment
- Social
- Daily living
- Literacy

Who is Eligible?
- Young Adults over the age of 18 who have a developmental disability.
- Must reside in Brantford/Brant County and live at home.

With supports provided in being involved in the Yound Adults in Transition program I was able to achieve my G2 drivers license and buy my own car!

How will young adults benefit?
- Young adults with a developmental disability will be actively involved in their community following graduation from school.
- Young adults will make informed choices about their own life.
- Young adults will build a sense of competence in decision making, accessing resources in their community and self-advocacy.
