-to facilitate in their transition from school to community life (including accommodation, work, leisure activities, and further education).
-to facilitate life planning and implementation of goals.
-to facilitate connections with the community
-to ensure appropriate supports and services.
-to provide
education/skill training (individual and group) in:
- pre-employment
- social
- daily living
- literacy
Who is eligible?
- Young Adults, age 18-28 who have a developmental disability
- Must reside in Brant County and live at home
How do you access the transition program?
- Referral must be made through Contact Brant, the central intake agency.
- Call Contact Brant at 519-758-8228.
How will young adults benefit?
- Young adults with a developmental disability will be actively involved in their community following graduation from school.
- Young adults will make informed choices about their own life.
- Young adults will build a sense of competence in decision making, accessing resources in their community and self-advocacy.
