-to facilitate in their transition from school to community life (including accommodation, work, leisure activities, and further education).

-to facilitate life planning and implementation of goals.

-to facilitate connections with the community

-to ensure appropriate supports and services.

-to provide education/skill training (individual and group) in:
  • pre-employment
  • social
  • daily living
  • literacy

Who is eligible?

  • Young Adults, age 18-28 who have a developmental disability
  • Must reside in Brant County and live at home

How do you access the transition program?

  • Referral must be made through Contact Brant, the central intake agency.
  • Call Contact Brant at 519-758-8228.

How will young adults benefit?

  • Young adults with a developmental disability will be actively involved in their community following graduation from school.
  • Young adults will make informed choices about their own life.
  • Young adults will build a sense of competence in decision making, accessing resources in their community and self-advocacy.
